Gabapentinoids (gabapentin, pregabalin) are used for nerve pain in sciatica when NSAIDs and simple analgesics are insufficient, targeting the neuropathic component of radicular pain.

How it works

Binds voltage-gated calcium channels in dorsal horn neurons, reducing neuropathic pain transmission from the compressed nerve root. Reduces the burning and shooting pain characteristic of radiculopathy.

What to watch for

Prescription only. Common side effects include sedation, dizziness, and cognitive dulling. Should not be driven until tolerance to sedation is established. Risk of dependence with long-term use. Not appropriate for first-line management.
