Methotrexate is the anchor disease-modifying antirheumatic drug (DMARD) for rheumatoid arthritis, recommended as first-line treatment by all major rheumatology guidelines. It slows or halts joint damage.
How it works
Inhibits dihydrofolate reductase, reducing lymphocyte proliferation and pro-inflammatory cytokine production. At the low doses used in RA, it has powerful anti-inflammatory and immunomodulatory effects.
What to watch for
Prescription only — requires specialist initiation. Regular blood monitoring (LFTs, FBC) required. Folic acid supplementation reduces side effects. Strictly avoid alcohol. Teratogenic — not safe in pregnancy.
