Probiotics restore disrupted gut microbiota, compete with pathogenic bacteria for adhesion sites, produce antimicrobial compounds and modulate intestinal immune response.
Most evidence is for infectious diarrhoea in children and antibiotic-associated diarrhoea. Strain specificity matters β Lactobacillus GG and S. boulardii have the strongest evidence.
Cochrane review of 63 trials shows probiotics reduce diarrhoea duration by approximately one day and reduce risk of diarrhoea lasting more than 4 days by 59%.
