How it works

Proposed mechanisms include diffuse noxious inhibitory control (counter-irritation), endorphin release, and modulation of descending pain inhibitory pathways.

Effects are modest and short-term. Real acupuncture is superior to no treatment but inconsistently superior to sham acupuncture. Best used alongside active therapies.

Why moderate evidence

Cochrane review found acupuncture moderately superior to sham and no treatment for chronic low back pain. Endorsed in NICE and ACP clinical guidelines.
