Low-dose amitriptyline and nortriptyline are effective first-line treatments for chronic neuropathic pain, particularly postherpetic neuralgia and peripheral neuropathy.
How it works
Inhibit reuptake of serotonin and noradrenaline in descending pain inhibitory pathways, enhancing the brain's endogenous analgesic system. At low doses (10–75mg), analgesic effects are independent of antidepressant activity.
What to watch for
Prescription only. Low analgesic doses are much lower than antidepressant doses. Side effects include dry mouth, constipation, sedation, and urinary retention — worse in elderly. ECG monitoring if cardiac risk factors present.
