Carbamazepine is the first-line pharmacological treatment for trigeminal neuralgia and is effective in approximately 70% of patients initially, significantly reducing attack frequency and severity.

How it works

Blocks voltage-gated sodium channels in hyperexcitable trigeminal nerve fibres, reducing ectopic discharge frequency that produces the characteristic electric-shock pain.

What to watch for

Prescription only. Common side effects include dizziness, sedation, and nausea. Requires blood count monitoring (rare aplastic anaemia). HLA-B*1502 genetic testing recommended in South Asian/East Asian populations before starting.
