How it works
Mechanism not fully established. May act on serotonin receptors and opioid pathways involved in temperature regulation rather than through oestrogen-like activity.
Not recommended for those with liver disease or hormone-sensitive cancers. Rare but serious liver reactions reported — discontinue if yellowing of skin or eyes occurs.
Cochrane review found black cohosh modestly reduces hot flush frequency. Evidence is mixed but generally positive for short-term use (up to 6 months).
