Gabapentin and pregabalin are widely used for postherpetic neuralgia, diabetic neuropathy, and other neuropathic pain conditions where carbamazepine is poorly tolerated.

How it works

Bind alpha-2-delta subunit of voltage-gated calcium channels in dorsal horn neurons, reducing calcium-dependent neurotransmitter release and decreasing neuropathic pain signalling.

What to watch for

Prescription only. Common side effects: sedation, dizziness, oedema, and cognitive dulling. Risk of dependence with long-term use. Evidence strongest for postherpetic neuralgia and diabetic neuropathy.
