Paracetamol (Acetaminophen)

for Tension Headache

The most widely used first-line OTC treatment for tension headache — effective, well-tolerated, and suitable for most adults including those who cannot take NSAIDs.

How it works

Paracetamol inhibits central prostaglandin synthesis via the COX pathway in the CNS, reducing pain signal transmission without the peripheral anti-inflammatory effects of NSAIDs.

What to watch for

Do not exceed 4g per day (lower with alcohol use or liver disease). Less effective than ibuprofen for severe tension headaches in some studies. Avoid overuse (>10 days/month) — risk of medication-overuse headache.
