Prazosin is an alpha-1 adrenergic blocker that reduces trauma-related nightmares and sleep disturbance in PTSD — one of the most distressing and persistent symptoms.
How it works
Blocks noradrenaline in the brain during sleep, reducing REM-stage hyperarousal associated with trauma nightmares. Lowers the sympathetic storm that drives nightmare content.
What to watch for
Prescription only. Can cause orthostatic hypotension (dizziness on standing) — take at night and rise slowly. The largest RCT (2018, NEJM) showed mixed results; earlier studies were more positive.
