How it works
Capsaicin activates and then desensitises TRPV1 receptors on C-fibre nociceptors, causing depletion of substance P and temporary defunctionalisation of pain fibres in the skin.
OTC low-concentration cream (0.025–0.075%) requires 4–8 weeks of regular application and causes initial burning — warn patients this is expected. High-concentration 8% patch (Qutenza) is clinic-applied and provides ~12-week relief.
